River Valley Community Health is pleased to offer a monthly membership plan for our healthcare services.
This plan is intended to offer flexibility and affordability for uninsured or underinsured patients.
This is a monthly fee to be paid at the beginning of each month.
Your membership will provide you unlimited (max of one visit per day) visits to our clinic.
All Level 1 and Level 2 office visits (as described in price list) and a select set of routine in-house labs are covered at 100% in the plan.
Should you need one of the many procedures we offer they will be deeply discounted. (see procedure list and membership fees for additional details).

Durable Medical equipment and medications administered in-house will also be discounted but charged separately. Prices will vary.
Services NOT covered under Membership
These are common examples of excluded services but should not be considered a comprehensive list. If at any time our provider determines that we are unable to safely provide the level of care you require or you need outside treatment, we fully reserve the right to refuse service if it is in the best interest of the patient. We highly advise patients to maintain health insurance should you need to see a specialist, require medical transport, emergency care, inpatient care, or any services we cannot provide at our facility.
Fever in a newborn less than 3 months old or fevers over 104 degrees
Other care best handled in the ER, hospital, or by a specialist
X-rays or other imaging
Acute cardiac emergencies or stroke Severe mental illness
Deep wounds and compound fractures Allergy testing
Heavy, uncontrollable bleeding
Serious head, neck, or back injuries Respiratory Emergencies
Severe Abdominal Pain
Lab work that cannot be conducted in- house
Gynecology and OB - Pregnancy and related conditions, like infertility
